For years, medications known as GLP-1 receptor agonists (GLP-1 RAs)—including drugs such as semaglutide (Ozempic®, Wegovy®) and tirzepatide (Zepbound®, Mounjaro®)—have been used to help people manage obesity and type 2 diabetes. Now, a major new study suggests these medications may offer an additional benefit for some women diagnosed with breast cancer: improved survival and a lower risk of recurrence.
Why This Study Matters
Obesity and type 2 diabetes are well-established risk factors for poorer breast cancer outcomes. Women with these metabolic conditions are more likely to experience recurrence, disease progression, and premature death. Because GLP-1 medications effectively improve blood sugar control and promote substantial weight loss, researchers have wondered whether they might also influence cancer outcomes.
To explore this question, investigators analyzed electronic health record data from more than 841,000 women with breast cancer across 68 U.S. healthcare systems. After sophisticated matching methods were used to create comparable groups, researchers compared women who used GLP-1 medications with similar women who did not.
What Did They Find?
The results were striking.
Among women with breast cancer and obesity, GLP-1 use was associated with:
- Lower overall mortality
- Lower risk of cancer recurrence or metastasis
- Better long-term outcomes over a 10-year follow-up period
Among women with breast cancer and type 2 diabetes, GLP-1 users also experienced significantly better outcomes compared with women treated with insulin or metformin.
The study found that GLP-1 users had substantially lower hazards of both death and recurrence, suggesting that improving metabolic health may play an important role in cancer survivorship.
What Could Explain These Benefits?
Researchers believe several mechanisms may be involved:

- Improved insulin sensitivity

- Better cardiovascular health

Laboratory studies have already shown that GLP-1 therapies may suppress tumor growth and influence immune and metabolic pathways involved in cancer progression.

Metabolic health matters.
Cancer survivors who improve body composition, reduce excess body fat, increase muscle mass, and improve insulin sensitivity may create a biological environment that is less favorable for cancer progression.
Exercise remains one of the most powerful tools available to improve these same metabolic pathways. Regular physical activity helps reduce obesity, preserve muscle mass, improve glucose control, decrease inflammation, and enhance cardiovascular fitness—all factors linked to better cancer outcomes.
Rather than viewing GLP-1 medications and exercise as competing approaches, future cancer care may increasingly use them as complementary strategies to improve survivorship outcomes.
Important Limitations
This was an observational study, not a randomized clinical trial. Therefore, the findings demonstrate an association rather than proof that GLP-1 medications directly caused the improved outcomes. Additional prospective clinical trials are needed before definitive treatment recommendations can be made.
The Bottom Line
This study provides encouraging evidence that GLP-1 receptor agonists may help improve survival and reduce recurrence among women with breast cancer who also have obesity or type 2 diabetes. While more research is needed, the findings highlight the growing importance of addressing metabolic health as part of comprehensive cancer care.
