Stronger for Longer: Resistance Training May Be One of the Most Underused Tools for Longevity—and Cancer Survivorship

A major new long-term study offers another compelling message for the Exercise Oncology movement: strength matters—not only for quality of life, but potentially for survival. 

Researchers followed 147,374 adults for up to 30 years, repeatedly measuring resistance training and aerobic activity patterns over time. Unlike many earlier studies that measured exercise only once, this study tracked behavior longitudinally—making the findings especially meaningful for long-term health and survivorship discussions. 

The Headline Finding: More Is Not Always Better

Compared with individuals performing no resistance training:

  • 90–119 minutes/week of resistance training was associated with:
    • 13% lower all-cause mortality 
    • 19% lower cardiovascular mortality 
    • 27% lower neurological disease mortality 

Interestingly, benefits appeared to plateau above approximately 120 minutes/week, suggesting that moderate, sustainable resistance exercise may provide the greatest return. 

For cancer mortality, the relationship was different:

  • Lower resistance training volumes (1–59 minutes/week) were associated with reduced cancer mortality. 
  • Higher volumes did not appear to further improve cancer-specific outcomes in this cohort. 

Why This Matters for Exercise Oncology

This study was not conducted specifically in cancer patients, but its implications for Exercise Oncology are difficult to ignore.

Cancer survivors frequently face:

  • Accelerated muscle loss (sarcopenia) 
  • Treatment-related fatigue 
  • Functional decline 
  • Cardiometabolic disease 
  • Reduced physiologic reserve 
  • Long-term neurologic and cognitive concerns 

Resistance training directly addresses many of these vulnerabilities.

Exercise Oncology has historically emphasized aerobic activity—and appropriately so—but this paper supports a more balanced message:

Muscle preservation and muscle building may deserve equal attention.

For cancer survivors, resistance training may help:

  • Preserve lean body mass during and after treatment 
  • Improve treatment tolerance 
  • Support Metabolic Health 
  • Improve independence and physical function 
  • Counteract frailty and aging acceleration 
  • Potentially contribute to improved long-term outcomes 

The Most Powerful Combination: Aerobic + Strength Training

One of the strongest findings was not resistance training alone.

The lowest mortality risk occurred when participants combined aerobic activity with resistance training. Participants performing both types of exercise experienced substantially greater reductions in mortality risk than either modality alone. 

This aligns closely with a central principle of CancerFitness.org:

Exercise is not one thing. The biology of cancer survivorship likely responds best to a combination of cardiovascular conditioning, muscle strengthening, mobility, and long-term consistency.

What Cancer Patients and Survivors Can Do Today

If medically cleared:

Add 2–3 resistance sessions per week
Prioritize major muscle groups
Combine with regular aerobic movement
Focus on consistency—not extreme training volumes
Track strength and body composition over time

Exercise should increasingly be viewed not simply as rehabilitation—but as an important component of survivorship care.
